Incident Report

Record and track workplace incidents efficiently

Every organization faces occasional incidents — from minor accidents to serious safety hazards like fire, electrical issues, or equipment failure. Properly documenting these events is critical for:

  • Preventing recurrence of similar incidents

The Incident Report module in Office-X allows employees and managers to report incidents easily and ensures management can track, review, and resolve them promptly.

Add a New Incident Report #

Employees or managers can create a new incident report by filling out the following information:

Incident Details*Detailed description of what happened
Reported BySearch and select employee
Incident ManagerSearch and assign employee
AttachmentsUpload supporting files

Click “Add” to save the incident report. 🎉

Incident Report List #

Once added, all incidents are displayed in a List of Incidents, where you can search, filter, and track reports efficiently.

You can also search by incident name or apply filters to quickly locate specific reports.
